Complete the Inquiry of Service Form
Please complete the Inquiry of Service Form by clicking the icon below.
You will be redirected to a secure external page to fill out the form.
Once submitted, our team will review your information and verify your insurance to determine your eligibility and coverage with our practice.

Receive the Intake Form Packet
After reviewing your Inquiry of Service Form and verifying your insurance, we will send you our Intake Form Packet via email.
This packet includes:

A comprehensive health questionnaire covering both mental and physical health history

Office policies and Notice of Privacy Practices

Release of Information (ROI) forms

Schedule Your Initial Consultation
Once you have completed and returned the Intake Form Packet, our team will review the collected information and reach out to you to schedule your first consultation.
The intake visit is a one-and-a-half-hour appointment. During this time, we will discuss your health history, goals, and any immediate concerns to create a personalized care plan tailored to your needs.
